Description

We provide young golfers with a structured & supportive environment to learn and develop their golf skills, while also imparting important life lessons & values. Group Coaching can be a fun and rewarding experience for juniors as they pursue their passion for golf.

Stephen McIver is hereinafter referred to as the PGA Professional

I the undersigned confirm that I am the parent/legal guardian of the child named below.

I hereby consent to the said child attending at Hurlston Hall Estate for the purpose of instruction in golf activities and training.

I confirm that such activities and training shall be in line with the Code of Ethics & Good Practice for Children’s Sport.

I confirm that the details I have provided including the medical details are correct and acknowledge that you are relying on the information provided in entering into the position of golf instructor to the child.

I undertake to inform the PGA Professional of any changes to this information.

I warrant and confirm that I am able to legally provide parental consent for the child to participate in the activities specified.

I acknowledge that the PGA Professional does not accept liability for any loss or injury to the child during the period of instruction unless such loss or injury shall be directly attributable to the PGA Professional.

I the undersigned will indemnify and keep indemnified the PGA Professional from and against any and all loss, damage or liability to the PGA Professional, Hurlston Hall Golf Club, its members, employees, guests and invitees arising from the actions or omissions of the child during the instruction period or whilst the child shall remain on the premises of Hurlston Hall Golf Club.

I consent/do not consent to the below mentioned child being included in any photographic or video material, in any publications/websites/social network applications which may be used for the purpose of documenting and highlighting their involvement in the golf activities and training.
